Tax Service Waiver Form

I understand that all tax filings, such as Federal, State and/or Local Taxes, submitted by Sandra’s Freelance Accounting LLC, are based on Client provided information. That Sandra’s Freelance Accounting LLC will file my tax returns as “Self-Prepared” and that neither Sandra’s Freelance Accounting LLC nor the Client (Tax Filer) will be signing them. Lastly, that my Tax Returns will not be filed without the full approval from me, the Client (Tax Filer)*

I further understand that Sandra’s Freelance Accounting LLC, is considered a “middle person” who is only helping the Client to file his/her taxes as if they were filing on their own. Sandra’s Freelance Accounting LLC commitment is to process the Client’s taxes to the best of her ability to get the best results for the Client, without violating any laws, keeping ethics intact and will follow the general accounting principles.*

Sandra’s Freelance Accounting LLC will not assume any responsibility for any tax penalties and/or interest resulting from the incomplete or incorrect information provided.*
